A few things to keep in mind when scheduling your first visit

Does my paperwork need filled out before my first visit?

We request that your new patient paperwork is completed online before coming to your appointment! If paperwork is not complete, be prepared to have your visit rescheduled since our office is very active! If you cannot access the paperwork, please come 30-45 minutes early to complete your forms. Paperwork will be texted and emailed to you after your appointment is scheduled.

Do I need to pay prior to my first visit?

We request payment to hold your appointment’s time slot. Payment invoice will be emailed to you after you book your appointment.

What if I need an appointment sooner than what's available?

If you need an appointment sooner than what shows available online, please call our office at (614) 781-8808.
